A Clinico-radiographic Evaluation of Wound Healing and Bone Formation after Surgical Removal of Impacted Third Molars Using Platelet Rich Fibrin (prf) and Prf Impregnated with 2% Metronidazole

Aim and objective: The aim of the study was to compare the efficacy of Platelet Rich Fibrin with or without 2% Metronidazole in mandibular third molar surgery by assessing the postoperative pain, swelling, wound healing (by wound infection and dehiscence ) comparing the quantity of bone healing at various time intervals Materials and Methods: Twenty patients aged 20-40 years who visited the Department of Oral and Maxillofacial Surgery, DayanandaSagar College of Dental Sciences were randomly selected and divided into a Control group and a Study group of ten patients each. Clinical assessment was done for both groups for wound dehiscence, wound infection, swelling (by using a modification of the Scultz-Mosgau method) and pain (by Visual Analog Scale) after 3 rd post-operative day, 7 th post-operative day and 14 th post-operative day. The radiographic assessment of mean bone height was done by using Orthopantomograph and evaluated preoperatively, immediate post-operatively and 3rd month post-operatively. Results: The VAS score of Study group (6.1 ±0.9) was lesser than the Control group (6.8 ±1.03) but the result was not statistically significant (p value was 0.14) on the 3 rd post-operative day. Swelling on 3rd post-operative day (3.48±0.5), 7th post-operative day (1.90±0.5) and 14th postoperative day (0.50±0.5) on the study group were consistently lesser than the control group on 3rd post-operative day (4.71±1.2), 7 th post-operative day (2.54±1.2) and 14th post-operatively day (0.81±1.2) but statistical significance value was found to be only on 3rd day (p value was 0.008). Conclusion: The Platelet Rich Fibrin with 2% Metronidazole group recorded reduced pain, swelling as well as enhanced and faster bone healing compared with those in the control group.
